Location:      741 52nd Street, Brooklyn, NY.  a)    A competent person did not inspection the scaffold after it was erected.  The 18 feet high tubular welded frame was erected on pieces of lumber spanning a stairwell, it had missing planks on the work platform, it did not have any means to access the work area and fall protection was not provide or erect on the 3rd tier.
