29 CFR 1910.147(c)(6)(ii): The employer's certification of periodic inspections did not identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection and/ or the person performing the inspection: a) CSWW, Inc. dba North 40 Outfitters, shipping / receiving area: On or about April 10, 2019 the employer failed to conducted the annual audit of the lock out / tag out program for the cardboard bailer, manufactured by the Philadelphia Tramrail Company, model 2800, and serial # 98C3989.
