OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fall hazards while operating the vertical reciprocating conveyor. Worksite: The employer did not ensure that the employees operating the vertical conveyor lift (Lift) were adequately protected from the hazard of falling into the shaft of the Lift at all times by maintaining proper communications with the outside service contractors so that the contractors would not improperly remove and/or fail to replace the drop pin from the initially installed interlock system. On or about April 11, 2019, an employee opened the door and fell into the vertical conveyor lift shaft, sustaining serious injuries from the incident. P.S.: Remove the No. 3) Recommended Abatement Method Utilizing ASME B20.1-2018 Safety Standard.
