29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by wearing protective helmets: On or about 04/10/2019, an employee of Norman Barahona dba Norman Barahona Roofing and Remodeling subcontracted by Select Renovations was not protected from a possible head injury. Tasked with picking up old roofing material that was being dropped from the roof, an employee without head protection; was exposed to a head injury from the falling debris.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: On or about 04/10/2019, employees of Norman Barahona dba Norman Barahona Roofing and Remodeling subcontracted by Select Renovations, tasked with tearing off and installing new roofing materials on a residential property located at 1414 and 1416 Northland Ave. in Lakewood, OH were not protected from a fall hazard of approximately 25 (+) feet by conventional fall protection and/or devices.
