29 CFR 1926.20(b)(1): A safety and health program was not initiated and maintained to provide compliance with the general safety and health provisions of the standard:          On or about 04/10/2019, the employer failed to develop, implement or utilize safety and health programs at the site location. Employees tasked with completing roofing activities were not trained on topics such as, but not limited to; how to properly don a lanyard and harness, safe ladder usage or fall hazards in construction (etc.).

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by wearing protective helmets:        On or about 04/10/2019, during tear off and re-roofing activities at a residential property located in Lakewood, OH, an employee without head protection was exposed to a head injury from falling shingles and/or other supplies located on the roof surface area above.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     On or about 04/10/2019, employees tasked with tearing off and installing new roofing materials on a residential property located at 1414 and 1416 Northland Ave. in Lakewood, OH, were not protected from a fall hazard by conventional fall protection and/or devices. Employees were exposed to fall hazard from a heights of approximately 25 (+) feet, from the top of the roof to ground level.
