29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On or about March 6, 2019 and at times prior thereto, the employer did not conduct frequent inspections or maintain programs to provide a workplace were any employee, worker, laborer or mechanic employed in the performance of the contract were free from recognized hazards such as but not limited to: a) employees performing roofing activities greater than 6 feet were not protected from falls; and b) employees performing roofing activities around open holes and sky lights were not protected from falls; and c) employees gaining access and exiting the roof areas were not protected from fall hazards where ladders were not extended at least 3 feet above the upper landing surface and tied off; and d) employees were not protected from head injuries when working below other workers with equipment and tools; and e) employees were not provided eye protection when using pneumatic nail guns; and h) employees were not provided personal protective equipment, fall hazard, and ladder safety training; and i) employees were not provided knowledge of hazard recognition awareness and how to avoid recognized hazards.
