29 CFR 1910.272(g)(1)(i): The employer did not issue a permit for entering bins, silos or tanks in the grain handling facility before the employees entered the bins, silos, or tanks: a) Grain Silos: On or about April 16, 2019 and at times prior, employees were exposed to acute illness, caught-in and amputation hazards from mechanical equipment, impairment of ability to self-rescue, incapacitation, and/or death from atmospheres such as, but not limited to; oxygen deficient atmospheres from off-gassing grain while conducting cleaning operations. The Employer failed to evaluate permit space conditions when entry operations were conducted by testing conditions in the permit space and issuing permits for bin entry.
