29 CFR 1910.253(b) (4) (iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: a. 116 O'Sheal Circle, Lavonia, GA (Outside of Shop Area) - Employees were exposed to fire hazard. The oxygen and acetylene cylinders used for welding operations were stored together without a noncombustible barrier between them, on or about 04/17/19.

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. Neither was the equipment approved for the class of location and for the ignitable or combustible properties of the specific gas, vapor, dust or fiber that would be present: a. 116 O'Sheal Circle, Lavonia, GA (Spray Painting Area) - Employees were exposed to fire hazard. The employer did not provide electrical equipment and/or wiring (fluorescent lights) as approved and/or intrinsically safe. The painter is using flammable paint in close proximity to the electrical equipment, which has accumulated paint (flammable) onto the electrical equipment and wiring, on or about 04/17/19.
