29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a- 73 & 75 Broadway Avenue, Lakewood NJ: Company worksite workers were exposed to struck by hazards when working beneath and around poured foundation erection and installation operations without the use of protective helmets. Condition observed on or about 04/15/19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    a- 73 & 75 Broadway Avenue, Lakewood NJ: A company employee was exposed to fall hazards of approximately 9 feet to the ground surface below when walking on top of forms used for a poured concrete basement without the use of fall protection.  Condition observed on or about 04/17/19.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
