(a) Sandtown Road Project, Excavation: On or about 04/16/2019, and at times prior, the employer exposed employees to cave-in hazards in an excavation that measured 6-feet, 7-inches deep, x 10-feet wide x 53-feet long, in that a cave-in protection system was not used for employees located inside an excavation engaged in sewer line installations.
