(a) No retention of OSHA injury and illness logs for calendar year 2017.  (b) No retention of OSHA injury and illness logs for calendar year 2018.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    101 Dan Fox Drive, Pittsfield, MA: On or about February 17, 2019, the employer had not reported to the Agency within 24 hours of a work related incident resulting in an in-patient hospitalization.  The Springfield Area office was notified of the hospitalization on April 9, 2019.
