29 CFR 1926.501(b)(14):     Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    a)  5669 Berkshire Valley Road, Oak Ridge, NJ.    Sheathing employees working on the second floor were exposed to falls of over 15 feet out the several window/wall openings to the ground below as the inside bottom edges of the wall openings were less than 39 inches at 32 inches.                                            Violation occurred on or about 04/16/2019.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      a)  5669 Berkshire Valley Road, Oak Ridge, NJ and at the Corporate Offices at 313 Lafayette Street, Apt. 3, Newark, NJ.    Employees exposed to fall hazards of, but not limited to falls from the sloped roof, were not provided instruction on fall hazard recognition and on procedures to minimize these hazards.                                            Violation occurred on or about 04/16/2019.

29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  5669 Berkshire Valley Road, Oak Ridge, NJ.    Employees placing roof sheathing were exposed to falls of over 22 feet from the 8:12 sloped roof as no methods of fall protection were in use.                                                                                Violation occurred on or about 04/16/2019.      Castle Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1335687, citation number 1, item number 2, and was affirmed as a final order on 02/26/2019, with respect to a workplace located at Building 20, Delano Court, Morristown, NJ.
