The discharge point of the shredder has an approximately 3-4 inch opening which exposes employees to rotating parts when trying to unjam the shredder.

The was most recently documented on 04/17/2019 at 210 Walnut Street, Room 347. A 10 amp. Sunbeam Toaster Oven, a 12 amp. Sharp Microwave Oven, and a 14 amp. Toastmaster Toaster were plugged into an SL Waber, Inc. relocatable power tap (AKA power strip) with a maximum load of 15 amps. (b) The employer is failing to protect employees from electrical and fire hazards while utilizing a personal electric heater. The was most recently documented on 04/17/2019 at 210 Walnut Street, Room 347. In cubical 03-17 a 10 amp space heater and computer equipment were plugged into a relocatable power tap (AKA power strip) with a maximum load of 15 amps. (c) The employer is failing to protect employees from electrical and fire hazards while utilizing a personal electric heater. The was most recently documented on 04/17/2019 at 210 Walnut Street, Room 347. In cubical 03-19, a 15 amp Pelonis space heater and computer equipment were plugged into a relocatable power tap (AKA power strip) with a maximum load of 15 amps.

(a) The employer is failing to protect employees from electrical and fire hazards. This was most recently documented during an inspection on 04/17/2019 at the site location of 210 Walnut Street, RM 347, Des Moines, IA 50309. A plug adaptor which interrupted the continuity of equipment grounding, was used to on an extension cord to connect a multi-outlet plug to power a television and cable box in the supervisors office.
