On or about April 9, 2019 at 3202 Northlake Pkwy NE, Atlanta, GA 30345: employees engaged in leading edge work in the alteration of a commercial building where employees went over the flags set up at six feet to dump garbage in a wooden dumpster raised to the roof by a forklift. Employees were exposed to falls from the roof edge up to approximately 14 feet and 2 inches to the concrete below.

On or about April 9, 2019 at 3202 Northlake Pkwy NE, Atlanta, GA 30345: employees engaged in leading edge work in the alteration of a commercial building where the flags did not extend the entire length of the in protected leading edge.
