29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about April 18, 2019, at the northeast exterior elevation of a commercial building under construction located at 3991 SE Federal Highway in Stuart, Florida, an employee was exposed to struck-by eye injuries when hand nailing and using a pneumatic nail gun to install 2.5 in steel nails while not wearing eye protection equipment.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about April 18, 2019, at the northeast exterior elevation of a commercial building under construction located at 3991 SE Federal Highway in Stuart, Florida, an employee was exposed to a fall hazard of approximately 16 feet when performing carpentry work from the basket of an aerial lift while not being protected from falling by the use of a harness and lanyard.
