29 CFR 1926.150(a)(2): Access to all available firefighting equipment was not maintained at all times:   Worksite located at 10841 N. Ambassador Drive, Kansas City, Missouri- Employees were exposed to fire hazards posed by the bonding adhesives that were being used on the roof and were not provided any means necessary to extinguish a fire if one were to break out on the rooftop.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:   Worksite located at 10841 N. Ambassador Drive, Kansas City, Missouri- Employees were observed working outside of the warning lines and were not be monitored to prevent falling. Employees were also exposed on the south side of the roof in which warning lines were not installed where employees were observed throwing materials off of the rooftop.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:   Worksite located at 10841 N. Ambassador Drive, Kansas City, Missouri- Employees exposed to fall hazards on a flat roof had not been trained on the recognition and avoidance of fall hazards and means necessary to mitigate the hazards.
