29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:  a) 1431 Mahone Ave., Bldg 9025, Fort Lee, VA 23801: On or about April 18, 2019, and times prior thereto, unused openings in a panel marked PA in a common hallway through a Military Clothing stock area, had knockouts missing in two locations, exposing employees to electrical hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a) 1431 Mahone Ave., Bldg 9025, Fort Lee, VA 23801: On or about April 18, 2019, and times prior thereto, a panel marked PA in a common hallway through a Military Clothing stock area, had a panel door that remained ajar due to a missing door lock that rendered the panel unguarded, exposing employees to electrical hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.304(f)(1)(viii): Circuit breakers used as switches in 120-volt and 277-volt, fluorescent lighting circuits were not listed and marked "SWD":  a) 1431 Mahone Ave., Bldg 9025, Fort Lee, VA 23801: On or about April 18, 2019, and times prior thereto, employees were required to turn fluorescent lights on in the morning and off in the evening using circuit breakers that were not listed and marked "SWD" in two panel boxes, exposing employees to electrical hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.32(b)(2)(ii): The annual summary did not include one or more of the following elements: the calendar year covered, the company's name, establishment name, establishment address, annual average number of employees covered by the OSHA 300 Log, and the total hours worked by all employees covered by the OSHA 300 Log:  a) 1431 Mahone Ave., Bldg 9025, Fort Lee, VA 23801: The employer combined the recordable injuries and illnesses on the OSHA 300A Summary log or equivalent form, from multiple stores on Fort Lee with the Military Clothing establishment, and did not record the number of employees and total hours worked at the establishment.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
