29 CFR 1910.138(a): The employer did not select and require employees to use appropriate hand protection when employees' hands were exposed to hazardous condition:    Establishment:  Employer is exposing employees to hazardous conditions. The employees are permitted to use inadequate gloves for handling hazardous chemicals.   On 04/18/2019 employees used examination gloves to handle hazardous chemicals such as, but not limited to, formaldehyde.  The employer did not ensure that the employees used chemical resistant gloves.

29 CFR 1910.1048(m)(1)(iii): Employer did not ensure that formaldehyde was included in the hazard communication program established to comply with 29 CFR 1910.1200:   Establishment: Employer is exposing employees to hazardous conditions.  The employer permits the employees to work with formaldehyde without providing the employees the required information through inclusion in the hazard communication program.   On 04/18/2019 employees worked with formaldehyde without information about formaldehyde as part of the hazard communication program and were not trained in accordance with the requirements of Hazard Communication regulation 29 CFR 1910.1200 and paragraphs (m) and (n) of 29 CFR 1910.1048.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   Establishment: Employer is exposing employees to hazardous chemicals.  The employer permits employees who are not trained in hazard communication to work where there are hazardous chemicals such as, but not limited to, cytotoxic chemicals and formaldehyde.                                                       On 04/18/2019 the employer did not provide hazard communication information and training to new employees in the past 2 years where there were hazardous chemicals such as, but not limited to,  cytotoxic chemicals and formaldehyde.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:    Establishment: Employer is exposing employees to hazardous conditions.  The employees are permitted to work where exit directional signs are not posted where the directions to nearest exits are not easily apparent.      On 04/18/2019 directions to nearest exits from the X-Ray room and the surgery areas were not apparent.  Exit directional signs were not posted.

On 04/18/2019 it was determined that employees who had medical and exposure records such as, but not limited to, workplace injury records and safety data sheets for chemical exposure, were not informed of the existence, location, availability of the records, and person responsible for providing access to the records.
