29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width by the use of a safety monitoring system alone:      a) Job site, 2020 Knotts Street, Pittsburgh, PA: On or about April 18, 2019, during roofing activities, the employer did not ensure that appropriate fall protection systems/devices were in use where employees were exposed to fall hazards of up to at least approximately 10 feet.

29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons:   a) Job site, 2020 Knotts Street, Pittsburgh, PA: On or about April 18, 2019, during roofing activities, the employer did not ensure that an appropriate system was implemented at the site to identify and correct safety hazards. Deficiencies noted during the inspection included, but were not limited to, the use of unsafe electrical systems and unlabeled/damaged ladders.
