The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

The employees were working on all areas of the roof and the employer did not ensure that they were trained in how to recognize and minimize fall hazards.

On or about April 11, 2019, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 5400 Riverview, Hinckley, Ohio 44233. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area.
