29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about April 16, 2019, employees were exposed to a struck by hazard while performing siding activities on a residential home located at 1537 Hopkins, Lakewood, Ohio 44107. There were employees working on the ground underneath employees on the upper levels and the employer did not ensure that they were wearing protective helmets.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about April 16, 2019, employees were exposed to a fall of approximately 12 feet while performing siding activities on a residential home located at 1537 Hopkins, Lakewood, Ohio 44107. Employees were working from ladder jack scaffolds and the employer did not ensure that they were using fall protection.
