Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: (a) On or about April 11, 2019 an employee removing a mold base off a cast shape, was exposed to pinch points created by the lifting loop and the mold base.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: (a) Management directed employees to use a locally fabricated lifting device attached to the tines of a Toyota Propane Forklift, Model 8FGCV25, serial number 52942, for below-the-tines lifting of mold bases and cast shapes, that was not approved by the manufacturer of the forklift.
