29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    A)     Mid-States Packaging, Inc., located at 12163 State Route 274, Lewistown, OH 43333: On or about April 19, 2019, the employer did not ensure that adequate periodic inspections of the energy control procedures for the 100 Ton Freeman Schwabe Automatic Press (Model # BP100-5979H; Serial # FHH1009) were conducted at least annually. Employees did not follow all the steps required for the control of the hazardous energy sources prior to conducting activities such as, but not limited to, attempting to find air leaks. Employees were exposed to hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to performing the tasks.

Employees did not control all hazardous energy sources such as, but not limited to, electrical before conducting activities such as, but not limited to, attempting to find air leaks. Employees were exposed to hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to conducting work.

Employees did not deenergize the 100 Ton Freeman Schwabe Automatic Press (Model # BP100-5979H; Serial # FHH1009) prior to conducting activities such as, but not limited to, attempting to find air leaks. Employees were exposed to the hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to performing the tasks.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    A)     Mid-States Packaging, Inc., located at 12163 State Route 274, Lewistown, OH 43333: On or about April 16, 2019, the employer did not ensure that the unused section of the blade was adequately guarded while employees operated the Tannewitz vertical band saw (Model: GHE 36; Serial # 84012), located in the expander/main room, to cut honeycomb material. During the band saw operation, employees' hands were in a close proximity to the unguarded blade, exposing them to amputation hazards.
