29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: A) Mid-States Packaging, Inc., located at 12163 State Route 274, Lewistown, OH 43333: On or about April 19, 2019, the employer did not ensure that adequate periodic inspections of the energy control procedures for the 100 Ton Freeman Schwabe Automatic Press (Model # BP100-5979H; Serial # FHH1009) were conducted at least annually. Employees did not follow all the steps required for the control of the hazardous energy sources prior to conducting activities such as, but not limited to, attempting to find air leaks. Employees were exposed to hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to performing the tasks.

During the band saw operation, employees' hands were in a close proximity to the unguarded blade, exposing them to amputation hazards.
