The employer did not ensure that employees were protected with eye and face protection, thus exposing employees to an eye injury, on or about 4/22/19.

The employer did not ensure that employees performing roofing work were provided with a personal fall arrest system, or other means of fall protection, thus exposing employees to a fall hazard, on or about 4/22/19.

The employer did not ensure that the extension ladder extended three feet above the roof's landing, thus exposing the employee to a fall hazard, on or about 4/22/19.
