29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 4/16/19, on apartment building construction site at 1166 Jefferson Avenue, Buffalo, NY: A portable electric heater was inside the locked company equipment trailer parked outside the building. The heater was connected to 120 volt-ac power via an extension cord that was plugged into a temporary receptacle inside the building and run outside and into the trailer. The plug on that extension cord lacked a ground prong. NO ABATEMENT CERTIFICATION REQUIRED
