29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:    JOB SITE: 221 Dobbins Road, Webster Groves, MO 63119, on or about 4/22/2019, workers were observed operating pneumatic nail guns without any form of eye or face protection.  The workers were exposed to struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     JOB SITE: 221 Dobbins Road, Webster Groves, MO 63119, on or about 4/22/2019, workers were observed on top of a 4:12 pitch roof, approximately 10' 3" above ground level, actively installing new roofing material and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.
