29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the length of the ladder:    (a) On or about March 26, 2019, employees were exposed to a 9 foot 8 inch fall while using a 24 foot Louisville metal extension ladder to access a 5:12 pitch roof.  The ladder was set up 18 feet high and 6 feet away from the top support of the ladder.    b) On or about March 26, 2019, employees were exposed to a 9 foot 8 inch fall while using a 20 foot Werner metal extension ladder to access a 5:12 pitch roof.  The ladder was set up 13 feet high and 5 feet 9 inches away from the top support of the ladder.
