29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width by the use of a safety monitoring system alone:      a) Job site, 1220 Western Avenue, Pittsburgh, PA: On or about April 22, 2019, during roofing activities on the stairwell roof, the employer did not ensure that appropriate fall protection systems/devices were in use where fall hazards of at least 50 feet existed in that there was no safety monitor system in use, no flag lines were in use, no one was tied off and parapet walls were not tall enough to serve as fall protection as they were determined to be approximately 16 inches in height.    DDP Roofing Services, Inc. was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1255893, citation number 1, item number 1, and was affirmed as a final order on February 22, 2018, with respect to a workplace located at 1320 East Grand Boulevard, Erie, PA, 16504.    DDP Roofing Services, Inc. was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1097138, citation number 2, item number 1, and was affirmed as a final order on February 16, 2106, with respect to a workplace located at 225 Washington Street, Conshohocken, PA, 19428.

29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, were not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:      a) Job site, 1220 Western Avenue, Pittsburgh, PA: On or about April 22, 2019, during roofing activities on the main roof of the building, the employer did not ensure that appropriate fall protection systems/devices were in use where fall hazards of at least 40 existed in that the parapet walls outside the flag lines were determined to range between 26 inches and 33 inches in height and no other form of fall protection was in use.
