29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Fredy Aguado, on or about April 22nd, 2019, at the location of 1983 Horseshoe Bend Dr., Perrysburg, OH, 43551:  The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Fredy Aguado, dba A&A Construction, on or about April 22nd, 2019, at the location of 1983 Horseshoe Bend Dr., Perrysburg, OH, 43551: The employer did not provide a training program for each employee exposed to fall hazards while employees were performing residential roofing activities.  Employees performing work in this condition were exposed to falls ranging from 9 feet to 19 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Fredy Aguado, dba A&A Construction, on or about April 22nd, 2019, at the location of 1983 Horseshoe Bend Dr., Perrysburg, OH, 43551: The employer did not ensure that employees engaged in roofing activities on a residential structure were protected from fall hazards. Employees were exposed to fall hazards ranging from 9 feet to 19 feet.
