29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    (a) Gutierrez Construction at 1913 Rolling Wind Windsor, CO 80550: On or before 4/23/19 the employer did not protect employees from injury in that a pump jack scaffold had decking stabilized between the uprights of an extension ladder where the distance between uprights and the decking was approximately six inches. This condition exposed employees to fall hazards.

The scaffold was deficient in its footing, decking and fall protection. This condition exposed employees to fall hazards.

The scaffold deck was 15 feet above grade. This condition exposed employees to a fall hazard of 15 feet.
