Production area - On or about April 19, 2019, authorized employees were required to perform servicing and/or maintenance on the Aquaflex printing press, Duesendury slitter, and other operating equipment. The employer did not ensure periodic inspections of energy control procedures were performed at least annually as required by the standard.

Production area - On or about April 19, 2019, the employer failed to ensure that adequate training was provided for every authorized and effected employees who work on or near the Aquaflex printing press machine, Duesenbury slitter, and other operating equipment. Employees were thereby exposed to hazard associated with rotating parts and ingoing nip points.
