29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). Jobsite at 285 Locust St., Woburn, MA On or about April 22, 2019, employees were exposed cave-in hazards while installing sewer piper that has straight and bench cut earthen walls without an adequate protective system.
