29 CFR 1910.134(c)(1)(ii): The written program did not contain provisions for medical evaluations of employees required to use respirators: a) Dietz Lofts on 225 Wilkinson Street, Syracuse, NY 13204, on or about 4/25/19: The written respirator program developed for employees wearing half face cartridge type air purifying respirators did not contain provisions for medical evaluations as provided in paragraph e of this standard including, but not limited to, procedures for filling out the questionnaire as contained in appendix C; identity and function of the physician or licensed health care professional (PLHCP) performing medical evaluations; supplemental information that must be provided for the PLHCP; follow-ups and medical examinations that may be required; and information on the ability to wear a respirator provided by the PLHCP to the employee and employer. Abatement certification must be submitted for this item.

The employee was not aware of the hazards of the product nor of the necessity for proper respirator fit. Abatement certification must be submitted for this item.
