29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a) At the production facility:  On or about 4/24/2019, an employee was attempting to increase the PH level in the water runoff system, by adding a corrosive material (caustic soda). The employee was splashed with the caustic soda in the eyes and face and the employer does not have suitable facilities for quick drenching or flushing of the eyes and body within the immediate work area.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    (a) In the outdoor enclosed area near the production facility:  On or about 4/24/2019, an employee was not provided effective training for handling caustic soda to treat water in the wastewater run-off tank and was injured when the chemicals splashed back into his face.

29 CFR 1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with either:  (i) The information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or,   (ii) Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.   (a) In the production facility:  On or about 4/24/2019, the employer did not ensure that a secondary container of caustic soda was properly marked and labeled, which would have provided the employees with specific information regarding the physical and health hazards of the hazardous material inside the container.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv):  (a) In the storage room:  On or about 4/24/2019, the employer had an extension cord that was used to supply permanent power to the buildings overhead door opener, and strung the cord across the support legs of the storage racks.
