29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a. On or about April 24, 2019, at the above addressed jobsite, an employee performing masonry operations from a pallet on rough terrain forklift, approximately ten (10) feet above the ground, was exposed to injuries from falls due to the lack of guardrails.   b. On or about April 24, 2019, at the above addressed jobsite, employees performing masonry operations from a tubular welded frame scaffold, thirteen (13) feet six (6) inches above the ground, were exposed to injuries from falls due to the lack of guardrails.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.   On or about April 24, 2019, at the above addressed jobsite, an employee working on the ground, below masonry activities, was not wearing a hard hat to protect against falling materials and debris.    Master Restoration, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1080549, citation number 1, item number 1 and was affirmed as a final order on December 2, 2015, with respect to a workplace located at 6010 N. Navarre Ave., Chicago, IL.
