29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : 1) Fast Back Operator, Line #3 - On or about 04/29/2019, operators were exposed to continuous noise at approximately 98.2% of the permissible daily noise level (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 89.7 dBA during the 428 minutes sampling period on April 29, 2019; exposure calculations included a zero increment for the 52 minutes not sampled. The employer did not obtain a new audiogram at least annually, including 2018 for each employee exposed at or above an 8-hour time-weighted average of 85 decibels. The employer had not provided the employee with an audiogram since September 11, 2017. 2) Packer, Line #3 - On or about 04/29/2019, operators were exposed to continuous noise at approximately 95.4% of the permissible daily noise level (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 89.7 dBA during the 469 minutes sampling period on April 29, 2019; exposure calculations included a zero increment for the 11 minutes not sampled. The employer did not obtain a new audiogram at least annually, including 2018 for each employee exposed at or above an 8-hour time-weighted average of 85 decibels. The employer had not provided the employee with an audiogram since September 11, 2017. 3) Packer, Line #3 - On or about 04/29/2019, operators were exposed to continuous noise at approximately 93.6% of the permissible daily noise level (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 89.5 dBA during the 469 minutes sampling period on April 29, 2019; exposure calculations included a zero increment for the 11 minutes not sampled. The employer did not obtain a new audiogram at least annually, including 2018 for each employee exposed at or above an 8-hour time-weighted average of 85 decibels.
