29 CFR 1910.23(b)(8): The employer did not ensure ladders were used only for the purposes for which they were designed:    On or about April 23, 2019, an employee used a 6 foot step ladder leaning against the wall  to climb over a parapet on the roof.  Employee was exposed to a fall greater than 4 feet.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    On or about April 23, 2019, an employee was on the roof of the building, removing the Christmas lights  and was not provided with any fall protection.  The employee was exposed to a fall greater than four feet.

29 CFR 1910.30(a)(3): In that an employee had not been trained in the nature and recognition of fall hazards in the work area; the procedures to minimize those fall hazards; the correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee uses; the correct use of personal fall protection systems and equipment.    On or about April 23, 2019, the employer allowed employee to do work on the roof of the building before providing training concerning fall hazards.
