29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about 4/22/19, during residential roofing activities, located at 37101 Elizabeth Lane in North Ridgeville, Ohio, the employer failed to ensure that employees utilizing nail and staple guns, while installing roofing materials, were provided eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:  On or about 4/22/19, during residential roofing activities, located at 37101 Elizabeth Lane in North Ridgeville, Ohio, the employer failed to ensure that employees were provided, and utilized fall protection. The slope of the roof varied from about 7/12 to 9/12, at a height of about 18'.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 4/22/19, during residential roofing activities, located at 37101 Elizabeth Lane in North Ridgeville, Ohio, the employer failed to ensure that the ladder used to gain access to the roof was extended at least 3' above the landing surface.
