29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a) On or about 04/24/2019, scaffolding was being erected by employees who had not been trained in the recognition and mitigation of scaffolding hazards. Furthermore, the competent person for scaffold erection was not on site as the scaffolding was being erected.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a) On or about 04/24/2019, employees were working off of a scaffold with a missing mid-rail exposing them to fall hazards of 39 feet. Employees were conducting stucco work.  b) Employees were working off of scaffolding with missing guardrails at the end of the scaffolding unit exposing them to fall hazards of 39 feet. Employees were conducting stucco work.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a) On or about 04/24/2019, employees working off of scaffolding were not trained to recognize scaffolding hazards, nor were they trained on procedures to control hazards.
