29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On 4/2/2019, workers were installing the roof of a commercial building located at 12001 North Forty Drive, Saint Louis, Missouri, 63131. Employees were installing shingles approximately 13 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards. On 4/2/2019, workers were installing the roof of a commercial building located at 12001 North Forty Drive, Saint Louis, Missouri, 63131. The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.
