29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   Worksite located at 6213 Oak St., Kansas City, Missouri- Employees were observed cutting and removing materials off the roof while using reciprocating saws, axes and pitch for to take up the materials and were not protected from eye injuries by the use of any means of eye protection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:   Worksite located at 6213 Oak St., Kansas City, Missouri- Employees were observed working on a flat roof without the use of any means of fall protection. Employees were exposed to unprotected edges as well as floor holes created by the contractor while removing the damaged roof deck.

Portable metal ladders and other portable conductive ladders may not be used near exposed energized lines or equipment:   Worksite located at 6213 Oak St., Kansas City, Missouri- Employees were exposed to a shock or electrocution hazard in that the employees were using an aluminum ladder near on overhead power line that was not insulated, isolated, or de-energized to prevent accidental contact.
