On or about March 27, 2019 the employer did not ensure that nominal 110 volt electrical power source (receptacle) was equipped with a GFI system. This condition exposed employees to hazards associated with electrical shock.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. On or about March 27, 2019 the employer did not ensure that flexible cords were inspected and removed from service when damaged or defective. This condition exposed employees to hazards associated with electrical shock.
