29 CFR 1910.109(e)(1)(iv): Persons authorized to prepare explosive charges or conduct blasting operations did not use every reasonable precaution, including but not limited to warning signals, flags, barricades, or woven wire mats to insure the safety of the general public and workmen:    a) Hydro Resources, County Road 18 and 69, Keenesburg, CO 80643:  On and before April 23, 2019, the employer did not ensure that persons authorized to prepare explosive charges or conduct blasting operations used every reasonable precaution to insure the safety of the general public and workmen.  The employer did not assess the area to ensure that extraneous sources of electrical energy such as, but not limited to, static electricity, video cable power, and a wireless surveillance system did not occur during the setup and blasting process.  In addition, the employer did not ensure that all personnel involved in the setup process were trained in safe working procedures, including controlling the detonation process by placing the initial charges downhole prior to making a connection.  Two employees were struck by shrapnel from explosive caps that detonated prematurely.  This condition exposed employees to struck-by hazards resulting in multiple shrapnel wounds from the discharges.

29 CFR 1910.109(e)(1)(vii): Due precautions were not taken to prevent accidental discharge of electric blasting caps from current induced by other sources of extraneous electricity:  a) Hydro Resources, County Road 18 and 69, Keenesburg, CO 80643:  On and before April 23, 2019 the employer did not take due precautions to prevent accidental discharge of electric blasting caps from current induced by other sources of extraneous electricity, including static electricity, video cable power, and a wireless surveillance system.  Two employees were struck by shrapnel from explosive caps that detonated prematurely.  This condition exposed employees to struck-by hazards resulting in multiple shrapnel wounds from the discharges.

29 CFR 1910.1200(f)(6):  Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked:    a) Hydro Resources, County Road 18 and 69, Keenesburg, CO 80643:  On and before April 23, 2019 the employer did not ensure that each container of Cotey Chemical Liquid Acid Descaler and Chlorine Bleach in the workplace was labeled, tagged or marked.  This condition exposed the employee to chemical hazards.    Abatement note:  One feasible and effective means of abatement is to comply with:      1)  29 CFR 1910.1200(f)(6)(i):  The information specified under paragraphs (f)(1)(I) through (v) of this section, for labels on shipped containers; or     2) 29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they are readily accessible during each work shift to employees when they are in their work areas:  a) Hydro Resources, County Road 18 and 69, Keenesburg, CO 80643:  On and before April 23, 2019 the employer did not maintain at the shop copies of the required safety data sheets for each hazardous chemical, and did not ensure that they are readily accessible during each work shift when on location to employees.  Employees were exposed to injurious corrosive materials, such as, but not limited to, soda ash, descaler, sodium hypochlorite, and bleach.  This condition exposed the employee to health hazards.

This condition exposed the employee to chemical hazards.
