29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: Worksite located at 6213 Oak St., Kansas City, Missouri- Employee was exposed to a shock or electrocution hazard in that the radial saw the employee was using had its' outer insulation breached and the inner wires were exposed.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: Worksite located at 6213 Oak St., Kansas City, Missouri- Employee was exposed to shock or electrocution hazards in that the extension cord being used on the site had its' grounding pin removed, thus the path to ground was not permanent and continuous. The other cord was plugged in to GFCI outlet, but the outlet was non-functional to protect employees from electrical hazards.
