Location: 577 West Main St., Hyannis, MA    On or about April 25, 2019 employees were exposed to struck by hazards from overhead work and falling construction debris, where hardhats were not worn.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: 577 West Main St., Hyannis, MA    On or about April 25, 2019 employees were exposed to fall hazards of more than 20 feet, from an upper level to the ground below, where personal fall arrest systems were not in use.
