29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.   On or about April 25, 2019, at the above addressed jobsite, an employee working on the ground, below roofing activities, was not wearing a hard hat to protect against falling materials and debris.   b.  On or about April 25, 2019, at the above addressed jobsite, employees working around a crane performing lifting operations, were not wearing hard hats to protect against struck by and falling materials.  C and V Roofing Corporation was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1353738, citation number 1, item number 2 and was affirmed as a final order on January 22, 2019, with respect to a workplace located at 426 Shadow Creek Dr., Palos Heights, IL.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about April 25, 2019, at the above addressed jobsite, an employee using a leaf blower did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.  C and V Roofing Corporation was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)91), which was contained in OSHA inspection number 1353738, citation number 1, item number 3 and was affirmed as a final order on January 22, 2019, with respect to a workplace located at 426 Shadow Creek Dr., Palos Heights, IL.

On or about April 25, 2019, at the above addressed jobsite, employees performing residential roofing operations, fourteen (14) feet ten (10) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  C and V Roofing Corporation was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1353738, citation number 1, item number 4 and was affirmed as a final order on January 22, 2019, with respect to a workplace located at 426 Shadow Creek Dr., Palos Heights, IL.
