29 CFR 1910.252(a)(2)(xv): Cutting or welding was permitted in areas that were not made fire safe by removing combustibles or protecting combustibles from ignition sources:  a) On or about 4/24/19 at Buffalo & Orchard Park Topsoil, Inc. located at 3340 North Benzing Road, Orchard Park, New York; an employee repairing a drive shaft from a truck was using oxyacetylene cutting torch. The work area was not free of combustible liquids or materials before cutting or burning commenced. The employee was severely burned when combustible liquids in the work area ignited engulfing the employee in flames.  ABATEMENT DOCUMENTATION REQUIRED
