Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: a) At the site, on the roof, on or about April 25, 2019, the employer did not ensure employees who working from a height of approximately nineteen feet while installing trusses, were protected from falls, thus exposing the employees to fall hazards.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, was not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a) At the site, on or about April 25, 2019, the employer did not visual inspect the electrical flexible extension cords used by employees to supply electrical power to the various hand tools used during the course of their workday.
