29 CFR 1910.147(c)(7)(iii)(A): Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures. a)On or about 4/22/2019 the employer did not ensure that an employee who left the company in May of 2018, returned in Jan/2019, the employee did not receive any retraining as an authorized employee to change a belt on a 10 hp. Caterpillar motor that drives the chain for the conveyor system in that, the employee opened the breaker panel, flipped off a single breaker, then began working on the belt that was not de-energized.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tag-out procedures) shall cover the following elements and actions and shall be done in the following sequence: a)On 4/22/2019 an employee who performed maintenance on a 10 hp. Caterpillar motor in the Line Bar area of the facility was not required to: (1)Turn off and shut down the Line Bar log feeding area. (2)Physically locate and operate energy isolating devices in such a manner as to isolate machine or equipment from the energy source. (3)Affix lockout or tag-out devices to energy isolation electrical device(s); and (4)verify that isolation and de-energization of the machine or equipment have been accomplished. Taylor Lumber Worldwide, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147 , which was contained in OSHA inspection number 1304473, citation number 1, item number1 and was affirmed as a final order on June 5, 2018, with respect to a workplace located at 18253 ST RT 73, Mc Dermott, OH 45652.
