29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Zeyon, Inc., Welding area, on or about April 26th, 2019 and times prior thereto: Employees utilized a Jet brand 6" bench grinder (Model # JBG-6A; Serial # 13060638) for grinding operations onsite. Employees were exposed to a struck-by hazard associated with the abrasive wheel on the left side of the bench grinder in that, the opening between the tongue guard and the abrasive wheel was measured at approximately 1 inch.

29 CFR 1910.215(d)(3): The contact surface(s) of wheel(s), blotter(s) or flange(s) on grinding machine(s) were not flat and free or foreign matter: a) Zeyon, Inc., Welding area, on or about April 26th, 2019 and times prior thereto: Employees utilized a Jet brand 6" bench grinder (Model # JBG-6A; Serial # 13060638) for grinding operations onsite. Employees were exposed to a struck-by hazard associated with the abrasive wheel on the left side of the bench grinder in that, the wheel was not flat and the center surface of the wheel contained excessive wear.
